Thrive HealthCare Services, a Home Health Care agency in Fairfax, VA (Servicing Northern Virginia) is seeking a PRN / contractor / or W2 Physical Therapist to provide services to our adult patients in the comfort of their homes. Fairfax County/ Prince William/ Loudoun County.
Job Description: We are looking for experienced and collaborative clinicians to provide outpatient PT services to patients. This is outpatient therapy delivered to the patient's home. The physical therapist should be able to demonstrate the use of variety evaluation and treatment approaches; customer service skills for interacting with patients, including communicating with patients and family members of diverse ages and backgrounds; and have effective communication skills. See patients during any hours/days of the week.
Job Requirements
* State-specific PT license
* Reliable, personal transportation for therapists practicing outside city centers
* 1+ years of experience
* Liability insurance
* CPR/BLS
* The Physical Therapist provides physical therapy services to patients, to include but not limited to screening and evaluation, treatment planning, treatment implementation, treatment re-assessment and revision, patient/client re-evaluation, and discharge planning.
* Demonstrate good verbal and written communication and organization skills.
* Knowledge of Kinnser and OASIS-D is a plus.
