We are seeking a dedicated and experienced Registered Nurse (RN) Supervisor to join our pediatric home healthcare team. This role involves providing skilled, complex nursing care and supervision to clients according to their Plan of Care and in alignment with IntegriCare’s philosophy, standards, and policies. The RN Supervisor will conduct in-home visits, supervise personal care aides, LPNs, and ensure the highest standard of care is provided.
Essential Functions:
- Provide skilled, complex nursing care and supervision to clients according to the Plan of Care and IntegriCare’s philosophy, standards, and policies.
- Participate in establishing and implementing the client's service plan in coordination with the staff providing the client's services.
- Regularly assess the nursing needs of the client.
- Conduct an initial face-to-face assessment of the client at the client's home.
- Work with clients and their families, assessing the home environment to determine their needs.
- Report problems and progress of the client to supervisors and personnel or the client's physicians.
- Perform annual assessments and evaluations of LPNs, PCAs, CNAs, Companions, or Sitters by direct observation or demonstration of tasks while the supervisees perform assigned tasks.
- Supervise the provision of personal care tasks for clients determined to be medically compromised, blind, or sensory impaired.
- Admit and assess clients for services and complete clinical documentation.
- Supervise visits when the provider receives a complaint concerning services at the supervisory home.
- Provide hands-on care, case management, and evaluation of the Plan of Care, and educate the client on proper care and treatment.
- Revise the plan in consultation with the physician based on ongoing assessments.
- Direct nursing and home healthcare as prescribed by the attending physician.
- Review all plans of treatments for accuracy, appropriateness, and completeness.
- Assist in the orientation of new caregivers and annual evaluation.
- Perform supervisory visits to each client to observe the caregiver and advise on proper care and procedures if necessary, and ensure client satisfaction with care.
- Assess and reassess pain adequately, utilize appropriate pain management techniques, and educate the client and family regarding pain management.
- Perform all aspects of client care in a safe environment that reduces the likelihood of medical or healthcare errors.
- Demonstrate knowledge of medications and their correct administration based on the age of the client and the client's clinical position.
Minimum Education & Experience Requirements:
- Bachelor’s degree in Nursing or related field.
- Current Georgia Registered Nurse (RN) license
- 2 years of medical supervision experience
- Home healthcare experience *nice to have*
- Valid driver's license
- CPR Certified
Knowledge, Skills & Abilities Required:
- Able to observe and assess the client visually and aurally.
- Able to demonstrate basic understanding and Proficiency ofworkplace software
- Able to effectively communicate in English with all levels of the workforce, both clinical and non-clinical.
- Able to perform and prioritize multiple functions or tasks.
- Meets applicable health requirements to provide client care.
Working Conditions & Physical Requirements:
- Work is normally performed in a client’s home.
- Able to constantly travel locally from assignment to assignment.
- Able to lift, reposition, and transfer clients.
- Able to stand, walk, bend, stoop, squat, kneel and reach freely and assist clients with walking.
- Able to work flexible schedule and/or evening hours as needed.
