POSITION DESCRIPTION
Position Title: Licensed Practical Nurse (LPN)
Department: Nursing
SECTION A: OVERALL PURPOSE OF THE POSITION
A Licensed Practical Nurse (LPN) is a nurse that has graduated from an accredited nursing program and is licensed as an LPN by the State of Hawaii. The LPN uses their training for performing quality nursing services for the care and treatment of patients in the offices.
SECTION B: MAJOR JOB RESPONSIBILITIES
- Performs general nursing duties, including escorting the patient to the exam room, preparing the patient for examination, taking and recording vital signs, observing symptoms, and reporting to the provider as necessary.

- Prepares patient records before visits, maintains accurate records of medications and treatments, and completes necessary requisitions.

- Directs and supervises other health care personnel in the clinic, delegating duties as appropriate.
PERCENTAGE: 20
- Screens phone calls to determine if appointments are needed and relays instructions from the provider to patients.
PERCENTAGE: 15
- Assists in other offices when directed and performs other tasks as assigned by the provider or nurse in charge.
PERCENTAGE: 10
- Maintains sanitary conditions in the office and orders medical and clerical supplies as required.
PERCENTAGE: 5
- Demonstrates quality work, completes assignments timely, and adheres to company policies and procedures.
PERCENTAGE: 2.5
- Maintains time and attendance standards consistent with company policy.

RESPONSIBLE TO: Director of Nursing
SUPERVISES: Clinic staff as applicable
MINIMUM QUALIFICATIONS:
Education/Experience: Graduate from an accredited School of Nursing; Licensure as an LPN in the State of Hawaii required; Active/unexpired CPR/BLS Card required.
Skills/Knowledge: Ability to manage multiple tasks; works overtime as needed; updates education independently; knows and uses principles of good body mechanics.
Desired/Preferred Qualifications: Effective communication and telephone skills; computer literacy preferred.
Medical Equipment: Familiarity with nebulizer, pulse oximetry, EKG machine, glucometer, and equipment specific to the clinic.
Characteristics: Physically fit for the duties; participates as a team member; possesses effective organizational skills and maintains a positive attitude.
Work Conditions: Walking (85%); sitting (15%); lifts 25 lbs. with occasional heavier loads.
Hourly Rate: LPN starting @ $26.06 / MA starting @ $23.72; based on experience.
