Job Description:
We are seeking a dedicated and compassionate Part-Time LPN to conduct comprehensive assessments of clients' health status, develop individualized care plans, train our team of professional caregivers, and collaborate with other healthcare professionals to ensure the delivery of high-quality care for our clients.
Responsibilities:
- Client Assessment and Care Planning:
- Conduct comprehensive assessments of clients' health status and develop individualized care plans.
- Coordinate with the interdisciplinary team to implement and modify care plans as needed.
- Client Education and Support:
- Educate clients and their families about health conditions, treatment plans, and self-care techniques.
- Provide emotional support and counseling to clients and their families to promote well-being and coping strategies.
- Documentation and Reporting:
- Maintain accurate and up-to-date client records, including assessments, care plans, progress notes, and medication administration records.
- Prepare and submit reports as required by regulatory agencies and company policies.
- Collaboration and Communication:
- Collaborate with other healthcare professionals, including physicians, therapists, and caregivers, to coordinate care and ensure continuity of services.
- Communicate effectively with clients, family members, and team members to promote collaboration and achieve positive outcomes.
- Quality Assurance:
- Implement and maintain quality assurance processes to ensure compliance with regulatory standards and company policies.
- Monitor and evaluate administrative procedures to identify areas for improvement and implement solutions.
Qualifications:
- Valid LPN license in the state of Connecticut.
- Minimum of 5 years of nursing experience, preferably in home care or a related setting.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities – verbal and written.
- Ability to work independently and as part of a multidisciplinary team.
- Commitment to providing compassionate care and promoting client well-being.
- Prior management experience, specifically coaching and motivating employees.
- Experience with geriatric population.
- Experience with intakes and initial assessments of prospective clients.
- Experience and comfortable with presenting in a large group setting.
- Knowledge of In-Home care rules and regulations.
