We are searching for Case Managers who will be responsible for working with justice involved persons through case management, assessments, goal planning and assistance services for approximately half of the clients in a residential reentry community correctional facility, along with clients in the nonresidential reentry community corrections program.
Pay ranges from $43,000-$48,000 annually
Monday-Friday - 1st Shift - 10 hour shifts
RESPONSIBILITIES
- Provide primary case management to a small caseload at Reentry sites in Ramsey County.
- Create a positive team approach to coordinated care and client impact as well as a collaborative partnership with support staff and other leadership to help facilitate a client-centered environment.
- Establish a robust network of referral sources and partners that will support the success of our clients.
- Cultivate and maintain relationships with community members, resources, and businesses in the community in which the Residential Reentry Facilities are located and within reason where clients are preparing to return upon reentry.
- Facilitate regular team meetings with Case Managers and relevant partners, participate in leadership and program planning meetings.
- Support and/or facilitate community centered programming at the Residential Reentry Facilities, supervise and provide group facilitation as needed.
- Attain, maintain, and promote a positive, safe, harmonious, and productive work environment.
REQUIRED QUALIFICATIONS
- High school diploma/GED.
- Current and valid driver's license, and a clean driving record.
PREFERRED QUALIFICATIONS
- Bachelor's degree in Criminal Justice, Corrections, Human Services or related field. Equivalent training and experience will also be considered.
- Excellent oral and written communication skills. Must be computer literate and able to navigate a Correctional Database system.
- Working knowledge or ability to learn the Criminal Justice System including correctional halfway houses, court, probation, supervised release, reentry programs and correctional systems.
