Summary
As a facility-based APP, your primary role will be to deliver top-notch Provider-level direct patient care within the Post-Acute Care setting. This position plays a crucial role in enhancing the clinical status and quality of life of our residents. This role is located in Leeds, MA.
Essential Duties and Responsibilities
- Uphold our company's care philosophy, focusing on maintaining or improving each patient's clinical status and life quality.
- Perform Annual Wellness Visits for long-term Nursing Facility residents and possibly Annual History and Physicals as required.
- Provide clinical interventional and episodic direct care visits, along with necessary re-evaluation of care.
- Ongoing evaluation and treatment of chronic, co-morbid illnesses.
- Respond promptly to changes in residents' conditions, reviewing medications, orders, and diagnostic tests.
- Coordinate care with collaborating physicians for overall medical management.
- Assist in managing facility-wide illnesses in collaboration with other disciplines.
- Maintain effective communication with families, participating in family conferences when possible.
- Execute other duties as required, adhering to the highest standards of care and professionalism.
Qualifications
- Valid nurse practitioner or physician assistant license in Massachusetts and current board certification.
- Bachelor's and Master's Degree in Nursing from accredited institutions.
- Experience in nursing with a preference for experience as a Nurse Practitioner/Provider.
- Proficiency in Nursing and clinical Nurse Practitioner critical thinking, particularly in aged, chronically ill, and convalescent care.
- Strong leadership, interpersonal, and problem-solving abilities.
- Excellent written and oral communication skills.
- Punctuality, organizational capabilities, and effective teamwork skills.
Additional Requirements
