Join our dedicated home health team as a Licensed Practical Nurse (LPN)! You’ll provide compassionate care to patients in their homes and collaborate with a multidisciplinary team.
Home Health LPN Responsibilities:
- Maintain professionalism and strong relationships with patients, visitors, and colleagues.
- Conduct patient revisits under the Team Clinical Manager's supervision.
- Collaborate with RNs and other team members to ensure coordinated care.
- Communicate effectively to promote patient-centered care.
- Demonstrate strong writing, computer, and organizational skills.
- Identify and address safety issues for staff and patients.
- Engage empathetically with a diverse patient population.
- Ensure accurate data collection and record-keeping.
- Present a neat and professional appearance.
- Adhere to attendance policies.
Home Health LPN Requirements:
- Minimum of one year of nursing experience; home care experience preferred.
- Must possess a valid driver's license and automobile insurance as required by law. Candidates must be at least 21 years old to meet fleet vehicle insurance policy requirements.
- LPN License and BLS Certification are required.
- Possession of professional liability insurance is encouraged.
