Primary Purpose of Position:
North Shore Community Health (NSCH) is an industry leader in exceptional community healthcare delivery. We have an opening for an Advanced Practitioner (Nurse Practitioner (NP) or Physician Assistant (PA) Access Provider). Under the supervision of the Clinical Manager of Patient Access, in collaboration with the Chief Medical Officer, the NP/Physician Assistant (PA) will provide continuing and comprehensive health care for individuals and families across all ages and genders. The NP/Physician Assistant (PA) will provide direct clinical service to patients, be involved in quality improvement activities, administrative activities, and help to develop programming as needed. Care may be delivered at any of NSCH locations or virtually via tele-health. As a team member at our health center, you will work with staff who are racially, ethnically, and culturally diverse, so being culturally competent is required.
Essential Functions:
- Collaborate with site leaders (including Medical directors, Practice Managers, and RN managers) to provide additional patient access, which could be in the form of vacation coverage or other planned time off.
- Manage the primary care providers’ in-basket, called “Inboxology”. The access provider will have dedicated time during the week to work on the primary care providers’ in-baskets to address test results, medication refills, and patient calls and messages. The access provider will generate billable encounters when performing Inboxology to meet their weekly patient productivity goal.
- Respond to emails and messages promptly.
- Close all billable encounters within 72 hours of the date of the visit.
- Complete care gaps and preventative care measures at every patient visit when possible.
- Meet the weekly patient productivity goal as discussed with the Clinical Manager of Patient Access and report the productivity number at the end of each week.
- Collaborate with Population Health to serve as a clinical content expert and provide access for the Population Health team when certain quality measures need to be met within a specific time frame.
- Collaborate with Operations and the Director of Patient Access to meet organizational goals and needs such as new patients.
- Provide direct care to scheduled patients and/or walk-in patients when necessary.
- Follow up on laboratory and imaging testing and communicate the results to patients in a timely manner.
- Obtain history and physical examinations of patients in an ambulatory setting.
- Utilize laboratory and other diagnostic services as appropriate.
- Make diagnoses and administer treatments based on evidence-based medicine, whenever possible but always within accepted standards of care.
- Seek outside advice and consultation as necessary; refer for specialty care and for confirmation of diagnosis and treatment when appropriate.
- Maintain strict adherence to the NSCH Confidentiality policy including HIPAA guidelines.
- Treat all patients in a welcoming and professional manner.
- Contribute to the team effort by supporting all team members and maintaining an open and positive attitude.
- Work as an integral member of the primary care team, with careful attention to customer service, population management, appropriate delegation, and participation in the quality initiatives of the health center. The advanced practice practitioner is expected to display exemplary communication skills and lead by example.

Requirements
- Master’s Degree in Nursing with certification as a NP or graduate of a certified Physician Assistant program.
- New graduates are welcome to apply.
- Current licensure to practice in Massachusetts.
- Experience in community health care.
- Must have reliable transportation to travel between sites.
- Preference will be given to bilingual/bicultural individuals who are Spanish or Portuguese.
- Ability to work effectively both independently and as part of a team.
- Strong communication and interpersonal skills, with commitment to establishing a safe and respectful environment for team members.
- Ability to work in a culturally diverse environment.
- High level of commitment to continuous improvement in the quest for comprehensive and quality health care.
- Completion of NSCH compliance trainings.
NSCH ensures that all employees comply with the annual requirements for vaccinations and health screenings. Physical Requirements:
This job is inherently sedentary, necessitating the ability to remain seated or standing for extended durations, while also utilizing computers and phones. There may be a requirement to travel to multiple locations throughout the day.
