Are you an experienced Licensed Practical Nurse (LPN) looking for a unique and rewarding opportunity? If so, then you could be the perfect fit for our home care company.
We are looking for a motivated and compassionate LPN Nurse to join our team. In this role, you will be responsible for admitting and monitoring clients (patients) in their homes, and also providing supervision, feedback and guidance to nursing assistants working in the home. You will assess patient needs, develop and implement nursing care plans, and monitor patient progress. You will also be responsible for communicating with other members of the care team and providing education to patients and their families. You will submit timely written (or electronic) home visit reports to your nursing supervisor every week.
Our ideal candidate is a licensed LPN with a minimum of one year of experience in a home health care setting. You will be a great communicator with excellent organizational and problem-solving skills. For this position, we ask that you commit to visiting 5 to 12+ clients every two (2) weeks, and have the ability to travel within a 50 mile radius. A valid driver's license and reliable transportation is required.

DUTIES
- Works to promote, maintain, and restore healthy living in the community for homecare clients and confer with your nursing supervisor RN regarding changes in the Plan of Care.
- Instructs and supervises through the educating of the home health aide, clients, and family members, the prevention of infection, accident, and injury.
- Completes an initial Admission Assessment and ongoing supervisory visits to monitor home care services.
- Assessment of the client's needs/problems at appropriate time points.
QUALIFICATIONS
- Current and unrestricted Georgia LPN licensure
- A minimum of one year nursing experience preferred
- Strong medical surgical, geriatric care experience preferred
- Home Health experience a plus
- Current CPR certification
- Good organizational and communication skills
- Available to travel within Laurens and surrounding counties.
- Timely report submissions
- Commitment to scheduling and visiting a minimum of 10 clients per month
