Your Role on the TeamThe candidate will be situated in the Emergency Department and occasionally on the inpatient wards, working with the nursing staff and physicians to help assist in the treatment and follow up on the above screening tests. The department has mental health and substance use advocates, for which the employee may also consult as necessary for holistic wrap around care for patients.
We are seeking a dedicated and compassionate Care Coordinator to join our Emergency Department team, specializing in providing support and assistance to patients with HIV, HCV, Syphilis, Chlamydia, or Gonorrhea. The ideal candidate will play a critical role in ensuring that patients receive comprehensive care and support during their visit to the emergency department and facilitate their journey through the healthcare system.

- Familiarity with healthcare software and electronic health records (EHR) systems Required
- Cultural competence and the ability to work with diverse patient populations Required
- Comfort with working with patients with potential infectious diseases Required
- Ability to analyze and take action on complex matters, documents, and tasks Required
- Ability to take the initiative and independently respond to requests in writing, in person, or over telephone Required
- Accurate typing skills, data entry skills, file organizational skills, filing skills, appropriate telephone skills and pleasant telephone etiquette, ability to carefully and accurately proofread own and other's materials Required
- Ability to work in a dynamic environment of rapidly changing priorities and frequent interruptions Required
- Ability to appropriately set priorities and work independently Required
- Ability to creatively solve technology problems Required
Required:
- Bachelor's degree in Nursing, Social Work, Public Health, or a related field.
- Current state licensure or certification, where applicable
Preferred:
- A master's degree
- 1-3 years Previous experience working in an emergency department, infectious disease care, or a similar healthcare setting.
