Position Summary:
Under the direction of the CORE Center Screening Clinic Manager and Director of Patient Centered Care, the Health Educator Lead will be the primary contact for PrEP in Screening Clinic. The Health Educator Lead will oversee PrEP service activities in Screening Clinic that include navigation of PrEP initiation and linkage; primary contact to handle and process Gilead Advancing Access applications; conduct follow-up calls to patients enrolled in the program; work closely with the Health Educators to address PrEP client’s health needs in a timely manner. The Health Educator Lead will follow-up on any PrEP referrals received from the Testing & PrEP Coordinator. The Health Educator Lead should maintain up to date knowledge of Pre Exposure Prophylaxis (PrEP), HIV and other Sexually Transmitted Infections (STI’s). The Health Educator Lead will perform pre and post HIV/STI testing and counseling to identify PrEP eligible clients; develop and implement group and client-level health education sessions related to PrEP, medication management, adherence, and treatment. The Health Educator Lead will participate in all administrative meetings and staff development meetings, representing health education and prevention issues, activities, goals, and outcomes. Flexibility working evenings and/or weekend hours as needed.
Duties and Responsibilities:
HIV/STI testing and counseling
- Perform patient centered behavior risk assessments and develop reduction plans and health education goals
- Perform rapid HIV testing through a finger stick
- Ensure proper collection, handling and disposal of HIV/STI
- Assess personalized Pre and Post HIV/STI testing, counseling and prevention education sessions to the patient’s individual health risks within screening clinic and in the community
- Provide patients HIV/STI results and maintain records of patients who need treatment and offer risk reduction strategies
- Provide excellent customer service when interacting with patients and internal/external customers
Health Education
- Disseminate HIV/STI education on prevention, transmission, adherence, treatment and other diseases including hepatitis, dietary needs, risk reduction, diabetes, hypertension and other health related topics.
- Provide group level health education training to patients, providers and community on PrEP medication management, adherence, and treatment.
- Distribute health education materials and contraceptives (e.g. condoms)
PrEP Linkage and Clinic flow
- Oversee and address any issues related to PrEP initiation and linkage
- Handle/process all Gilead Advancing Access applications
- Conduct follow-up calls to PrEP clients in between appointments and assist in scheduling a future appointment if necessary
- Assist the Health Educator to address any client concerns related to PrEP
- Provide proper follow-up to all PrEP referrals in a consistent and timely manner provided from the Testing & PrEP Coordinator
- Refer HIV positive patients to HIV medical care and other prevention programs as needed including DIS for partner services
- Initiate high risk HIV negative patients who visit the Screening Clinic to PrEP services who qualify for PrEP at CORE Center and other CCHIP sites.
- Identify barriers affecting adherence to treatment and develop a follow-up care plan during the counseling session
- Communicate effectively across multiple disciplines the specific needs of patients
- Oversee PrEP clinic flow and provide recommendations to improve PrEP services and patient satisfaction
Administration
- Collect and enter HIV Counseling and testing activities into program database, Cemer medical record, CareWare and/or Provide on a daily basis
- Maintain record of Gilead Advancing Access applications in the MAP Group on Sharepoint
- Document phone calls, referrals including but not limited to PrEP referral services and prevention programs
- Submit monthly Report of Health Education activities to Screening Clinic Manager
- Collaborate with the Screening Clinic Manager, Director of Patient Centered Care and Director of Quality Improvement & Evaluation Quality, to assess PrEP program effectiveness in achieving stated goals; trouble shoot issues that arise for resolution
- Adhere to CORE Center and department specific policies and procedures
- Maintain open communication with supervisor (e.g., respond to phone calls, emails, faxes, and pages) and team members
- Coordinate with the Screening Clinic Manager to oversee inventory of testing supplies and other resources
Other:
- Screening Clinic Health Educator Lead is encouraged to participate in ongoing trainings in specialty areas for up-to-date information
- Provide clinic coverage/general coverage when shortage arises
- Participate in all CORE Center, CCHIP Domains and department mandatory trainings and meetings including monthly PrEP call meetings
- Serve as the lead in updating health education presentations and resources
- Complete other duties assigned by the Screening Clinic Manager
- COVID-19 Precautions: Protective equipment, face masks and shields are provided Remote work unavailable at this time; On-site work is required
QUALIFICATIONS
Bachelor’s degree in a health-related field, public health, education, psychology, social science, or a minimum of five years of counseling required. One to three years of experience providing HIV counseling and education is required. Partner Services training complete is a plus. Ability to work with a diverse population within a multicultural and multidisciplinary team. Experience with Microsoft programs and medical records preferred. Strong oral and written communications skills. Ability to meet deadlines and work as a team player. Candidates experienced in administering HIV/STI testing, counseling and providing PrEP services are strongly desired.
Flexibility to work evening and/or weekend hours.
PHYSICAL DEMAND/HAZARDS/ENVIRONMENTAL CONDITIONS
Able to lift 10-pound boxes. Computer screen exposure and wrist and hand movements.
