Position Summary:
The RN Coordinator Embedded Care – Oncology’s primary responsibility is to manage care for complex patients of the hematology/oncology practice. This individual will perform nursing intervention in a prescribed and direct way to meet patient needs and update and revise care plans as needed to achieve desired patient outcomes.
Primary Duties and Responsibilities:

Assessment/ Evaluation:
- Develop and monitor the care coordination process as a member of the oncology clinical team.
- Coordinate and monitor the care management process and treatment plans for selected patients in collaboration with providers.
- Assess patient educational needs and evaluate response/understanding to patient/family.
- Identify patients with co-morbidities and lack of psychosocial support and develop a proactive plan to promote safety, alleviate distress and reduce admissions.
- Perform telephone triage per organizational policy.
- Collaborate with payer’s case managers to facilitate case management for their members.
Planning/ Preparing:
- Collaborate with multidisciplinary team members to coordinate care across the cancer care continuum.
- Review provider schedules to assist with prioritizing visits and make recommendations to and/or consult with providers when provider schedules are full.
- Support and mentor medical assistants and assist with planning/preparing to ensure efficient and smooth patient flow for each clinic day.
Intervening/ Executing:
- Coordinate with patients and members of the oncology interdisciplinary team, and others outside of the practice as needed, to ensure appropriate patient/caregiver education and ensure that interventions (testing, procedures, consults) are scheduled with the Oncology Nurse Navigator.
- Empower patients to be an active member of their care team, working with patients/caregivers to develop an individualized plan of care.
- Educate new and existing patients in medication usage, side-effect management, adherence to IV and oral treatment plans, rationale for treatments and other interventions as needed, and modifying teaching methods to accommodate patient’s learning style and abilities.
- Assist provider with serious illness and end-of-life discussions.
Communicating/ Updating:
- Proactively communicate with providers, clinical staff, and others.
- Thoroughly document all interventions and outcomes on a timely basis in the patient’s medical record according to policy.
- Report occurrences through online portal as necessary.
- Demonstrate sensitivity and patience in responding appropriately to the verbal and nonverbal communication of patients, families, and co-workers.
- Check and respond to emails daily.
Compliance/Regulation (where applicable):
- Report occurrences utilizing the electronic occurrence reporting system.
- Understand and carry out the policies and procedures of the Medical Center.
- Adhere to HIPAA regulations and abide by confidentiality laws and regulations.
- Adhere to infection control and safety guidelines and is skilled in appropriate use of standard precautions, safe workplace, and confidentiality methods.
Professional Development:
- Maintain CEUs and attend pertinent educational offerings to maintain a high level of clinical expertise.
- Attend Tumor Board and Clinical Team meetings.
- Participate in Cancer Committee projects and special projects.
- Attend mandatory educational sessions and/or in-services, including but not limited to Triage, and Annual Clinical Skills Day.
- Maintain current knowledge of nursing, disease management and utilization management.
POSITION QUALIFICATIONS
Minimally Required
The minimum required to perform the essential duties of the role.

Education
Formal education (e.g. High School Diploma, Bachelor’s degree, graduate of an accredited program)
|
Associate of Science in Nursing required.
|
Bachelor’s degree in Nursing preferred.
|
Licensure/certification
Include due date if licensure/certification is required after hire.
|
Current NH license as a Registered Nurse required.
Certifications:
- Current BLS required.
- Receive ONCC Oncology Certification (OCN) upon two (2) years of hire required.
- Chemotherapy/Immunotherapy certification upon completion of orientation required.

ONCC Oncology Certification preferred.
|
Experience
Number of years and type of directly related work experience (e.g. 2-4 years of accounting experience)
|
A minimum of one-two years of med/surg, critical care, and/or hematology/oncology experience (ambulatory/inpatient) required.

Oncology experience (inpatient/outpatient) preferred.
