Nemours is seeking a Nurse Navigator (HemOnc), Casual, to join our Nemours Children's Hospital team in Orlando, Florida.
Focus will be on Hematology with Oncology coverage as needed.

Serves as a pediatric oncology and/or hematology nurse navigator. This position is responsible for a high level of service and quality of care and coordination to individuals requesting or receiving service for the HemOnc (Nemours Children's Center for Cancer and Blood Disorders, NCCBD) and sub-specialties where patients of HemOnc/NCCBD are treated. The position provides advocacy and education for complex patients, their families, and caregivers to overcome health care system barriers.
Assist with all aspects of navigating and provide education to assist the patient through the health care process assuring that continuity of care is promoted in all patient care settings. Collaborate with the physicians, other healthcare team members, and the patient/family to maintain a plan to deliver high quality healthcare in the outpatient setting and in the home environment, focusing on educational and resource needs.
This position is responsible for the following:
- Proactively develops and enhances processes to improve clinical experience for referred patients and the referring physician. Provides clinical assessment expertise to scheduling parameters - timing of visit, appropriate provider, etc. Improves access to care by assuring efficient and effective coordination between visits including scans, complex scheduling, sub-specialty interface, home care, and inpatient admissions.
- Coordinates multidisciplinary/comprehensive clinics, assisting with clinic flow and alerting providers to patient needs. Supports the work of research and serves as a critical interface.
- Coordinates and delegates prior authorizations and peer-to-peer.
- Participates in multi-disciplinary team discussions in relation to the individualized patient care plan. Bridging gaps in care and supporting care coordination for patients with highly complex care needs and serves as a liaison between the patient/caregiver and provider when necessary.
- Facilitates and follows-up on lab, scans and other tests; assists with specialty medication management; and provides education around those medications. Triages patient and families' complex needs including telephone and electronic encounters.
- Evaluates the patient/family response to care on an ongoing basis. Tracks patient outcomes and relevant data using information systems and pathways. Maintains any registries as needed.
- Promotes and provides patient, family, staff, and community education using clinical expertise and knowledge base in person, by phone, via newsletter, and through community outreach.
- Maintains Association of Pediatric Hematology Oncology Nurses (APHON) Chemotherapy/Biotherapy provider status.
- Serves as a resource and may provide cross-coverage in other areas including HemOnc Inpatient and Infusion Center in accordance with applicable job description requirements.
- Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee's supervisor.
Job Requirements
- Bachelor's Degree in Nursing required.
- Current Florida RN License in good standing.
- American Heart Association BLS Certification required.
- Minimum of one (1) year to three (3) years of experience required. Minimum two (2) years clinical experience, pediatrics HemOnc preferred.
- Obtain APHON certification within six (6 ) months of hire.
- CPHON, CPON, or BMCTN certification preferred.
- Travel to satellite locations may be required.
