Job Summary:
The LPN has the responsibility for providing direct and indirect nursing care to assigned participants; manages clinical situations for which he or she is responsible. The LPN will function within the clinic under the supervision of RNs. He or she will accept assignments dependent upon training and the availability of RN supervision. Duties include but are not limited to: completing daily medication passes and reviewing MARs, collecting data for assessments and care planning, evaluating participant responses to nursing intervention, completing clinical tasks as assigned by RNs, teaching and counseling as assigned by RNs, collaborating with other disciplines and assisting primary care providers with tasks and treatments. Other duties include communicating effectively with participants, families, staff, pharmacies, and vendors, accomplishing tasks in an orderly fashion, and working effectively in a team setting.
Specifications:
Education and Special Training: Graduate of an accredited school of nursing.
Experience: Has practiced nursing in the last 3 years. Three years LPN experience with at least 12 months experience working with the aging, frail, or elderly populations.
Licensure, Registry or Certification Required: LPN currently licensed in North Carolina (NC); presents credentials which demonstrate ability to meet performance criteria for LPN, Valid NC Driver’s license and vehicle.
Knowledge, Skills, and Abilities:
Must possess knowledge and skills necessary to treat participants. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by PACE of Guilford and Rockingham Counties and approved by Centers for Medicare and Medicaid Services (CMS) before working independently. With or without reasonable accommodations: Ability to bend and or stoop frequently; have independent mobility in order to climb stairs and walk distances. Lift and position patients of all weights and heights utilizing correct body mechanics. Lift and carry pieces of durable medical equipment. Lift and transport at least 50 pounds during the course of providing care, treatment, and/or services. Utilize correct body mechanics to transfer, lift, and position participants. Read, write, and speak in English. Communicate effectively both orally and in writing; ability to clearly see medical records and various documents. Hear blood pressures and spoken communication; ability to perform repetitive hand/wrist movements.
Key Responsibilities:
- The LPN is responsible for identifying the need for immediate intervention to address participant’s current status or condition. The LPN is not allowed to perform an initial, change of status or every 6 month assessment for the development of the individualized plan of care. The RN must perform this assessment per CMS regulations.
- The LPN will assist RNs in the care planning of client needs through the suggestion of goals and interventions by which the RNs will review and determine if appropriate.
- The LPN may implement the established plan of care with RN supervision. The LPN may not supervise, but may be assigned to assure that tasks to be carried out by unlicensed staff have been completed according to PACE policy and procedure.
- The LPN will participate in an evaluation by identifying client response to nursing intervention and suggesting to the RN revisions to the plan of care. The RN is responsible for follow up.
- The LPN will report and record her own data collection. He or she will sign his/her documented information. The LPN will participate in collaboration as assigned by the RN. The LPN must make the assigned RN aware of any disciplines he/she feels services will or may affect the patient’s health care. The LPN will participate in teaching and counseling of participants and their families as assigned by the RN by implementing an established teaching plan or protocol. The participants individualized care plan and informational sheets will be used as the guideline for the LPN to educate participants and caregivers. The LPN accepts responsibility for his/her self and the care he/she provides. The LPN will be held to asking questions/seeking clarification of expectations as needed; if unaware of how to perform a task, notify the RN. Communication must be a two-way process.
- Assist primary care providers with tasks such as med ordering and treatments as assigned.
- Reviews MARs and completes a daily med pass to participants in the ADHC.
- Communicates effectively with participants, families, staff, pharmacies, and other departments and agencies.
- Organizes, prioritizes, and accomplishes tasks in an orderly fashion.
- Facilitates achieving customer satisfaction and agency compliance by troubleshooting and helping to resolve concerns.
- Communicates any changes in participant condition to other assigned team members as required.
- Works effectively in a team setting.
- Maintains medical records according to policies and procedures.
- Abides by all established policies, rules, and regulations.
- On-call rotation as assigned by Clinical Coordinator.
