Position:Physical Therapist Consultant

Role:
The role of the PhysicalTherapist is to evaluate each resident’s mobilityskills, manage painand provide recommendations and plans to individual behavior support team that will support the individual’s independence, development, fall prevention, and self-direction.
The philosophy of care is team intensive, person centered, trauma focused, and addressed increasing replacement behaviors that promote community integration and living. The PhysicalTherapist Consultant will also over see and provide employee training and monitoring.
Orientation and guidance is provided.
Responsibilities:
- Assessment of each individual’s needs within the consultant's area of expertise.
- Participation in the interdisciplinary Individual Behavior Support Team’s (IBST) development of an Individual Behavior Support Plan (IBSP) for each individual based on the assessment data.
- The development of plans for IBSP objectives that are within the domain of the consultant's discipline and expertise.
- The consultant shall request the hours deemed necessary for their area in the IBST implementation.
- Consistent monitoring and evaluation of the delivery of services in the consultant's area to ensure that the program is being implemented properly and consistently.
- In-service training to staff regarding the discipline, areas treated, program objectives, and special techniques of the area, with ongoing follow-up to ensure proper implementation. Can be done in person or remotely.
- Ongoing documentation of client progress and documentation in the client record, and in the online Transdisciplinary Clinical Notes; may cross-reference to a consultation report.
- Document at least monthly progress reviews.
- Provide information required and recommendations to the facility's program design.
- Participation in the facility’s Continuous Quality Improvement System by submitting recommendations in writing.
- The consultant shall be available to meet with the IBST monthly as necessary when scheduled and attend special incident reviews if applicable to their domain.
- The consultant shall be available for scheduled QA reviews from regional center and Department of Developmental Services semiannually.
- All client assessments, recommendations, plans, and monitoring of progress will be provided in writing to the Administrator and documented in the individual’s client record.
- Other related functions within the consultant's area of expertise agreed to and requested from the Administrator or QBMP for client welfare.
- Must be able to attend in person or via remote video meeting, monthly interdisciplinary team reviews on the fourth Friday of every month during the afternoons from 12:00pm - 4:00pm. Reports may be submitted for meetings when on vacations, conferences, etc but 75% of the meetings must be attended.
- Observation hours must be provided when residents are home during afternoons, evenings or weekends and not during school hours.
Qualifications:
- Two years of experience assessing and implementing programs for individual’s with developmental disabilities and mental health or behavioral challenges.
- Registered Physical Therapist
- Maintains registration
- Eligible for regional center vendorization
- Experience working in a transdisciplinary team
Hours:
40 Hours per month or as determined in each Individual’s Behavior Support Plan by the support team.
More hours available upon request.
Starting Wage:
Negotiable depending on experience.
