At Somerby Mobile, the mission is to inspire and nurture successful aging and our multi-disciplinary rehabilitation team will play an important role in keeping residents as independent and healthy as possible. A rehabilitation provider since 1983, Tx:Team brings specialized Geriatric Physical, Occupational, and Speech Therapy Services to the Somerby Mobile community.
The details:
- Somerby Mobile has a fitness center and aerobic exercise pool, therapy space, and a collaborative working relationship with the onsite providers. Our model for therapy services includes 1:1 patient care and a therapy-led onsite fitness program.
- The beautiful campus includes three living options for Seniors: Independent Living, Assisted Living and Memory Care.
- Patient care schedule is Monday - Friday with no evenings, weekends, or holidays.
- Candidates must be driven to excite the community and educate residents on rehabilitation services.
The ideal candidate:

The requirements:
- Two (2) years of experience in a geriatric outpatient environment
- Experience with Dementia, Dysphagia, and Cognition
- LSVT LOUD Certification is a plus
- Certificate of Clinical Competence
The clinical responsibilities:
- Deliver One on One quality speech therapy services according to treatment plans for patients
- Document progress toward identified objectives
- Provide accurate and timely documentation for current patient caseload within EMR
- Create a positive work environment to promote teamwork and communication
- Conduct patient and family education
