Family Nurse Practitioner/Physician Assistant- FLOAT POOL

Role Summary
As a Family Nurse Practitioner (NP) or Physician Assistant (PA) at Nest Health, you'll provide comprehensive primary, acute, chronic, and pediatric care to patients of all ages and backgrounds, both in the home setting and virtually. You'll conduct physical exams, screenings, and lab tests, overseeing the health of families under physician supervision, and collaborating with specialists in nutrition and behavioral health. You'll be both a health practitioner and educator, delivering culturally congruent, non-judgmental care, and working closely with Medical Assistants trained as Community Health Workers who assist with home visits, screenings, and social services navigation.
What You’ll Do
- Collaborate: Work with the Nest Care Team in the home, fostering teamwork and shared decision making across clinical and non-clinical members, including MAs/CHWs, LCSWs, and other clinicians.
- Primary Care: Oversee a panel of families, providing longitudinal primary care, care management, and coordination through in-home visits and virtual care (video, phone, SMS).
- Perform Assessments: Conduct patient assessments, physical exams, and care management, including infant and well child exams.
- Provide Urgent Care: Handle urgent/acute medical visits and calls for family members on your panel to ensure timely and appropriate medical care, preventing emergency department visits or hospitalizations.
- Post-Discharge Care: Conduct post-discharge visits within 48-72 hours of discharge to reduce readmission risks, perform detailed medication reconciliation, and assure that long-term services are in place.
- Clinical Standards: Maintain clinical policies and standards, patient safety, documentation, SOAP notes and medical records.
- Diagnostics: Order, administer, and record diagnostic tests (e.g., blood tests, X-rays). Diagnose health conditions and illnesses, assess cases presented by your care team, prioritize patient needs.
- Develop Treatment Plans: Propose treatments for chronic and infectious diseases, prescribe medications, and educate patients about their conditions.
- Case Notes: Maintain, update, and escalate accurate case notes, monitor clinical quality gaps in care at the household level, and consult with the care team.
- Patient Education: Educate patients, families, and non-clinical staff on preventive care and wellness.
- EHR Utilization: Use Nest Health’s electronic health record and care management tools for data collection, documenting member interactions in the field, and communication with your team, members, and community resources.
- Team Meetings: Participate in daily team huddles and weekly case conferences to discuss health needs and establish health goals in members' Nest Action Plans (NAP).
- Process Improvement: Work with leadership and product teams to improve software and operational workflows.
This role reports to the Market Medical Director and works closely with operational leadership.
Skills & Competencies
- Experience providing clinical services to individuals with chronic medical and behavioral health conditions, and interest in serving complex, vulnerable populations.
- Demonstrated proficiency, prior experience, and/or willingness to train in clinical nursing skills such as wound assessment and care, blood drawing (venipuncture & phlebotomy), assessment and care plan reinforcement for common adult and pediatric chronic conditions such as diabetes, respiratory concerns, hypertension, congestive heart failure (CHF), depression, etc.
- Proven skills, knowledge base, strong judgment, and problem-solving skills for independent clinical decision making.
- Motivated to explore and test ways that technology can support your work and help drive the ongoing movement toward new and better care models.
Experience & Qualifications:
- Completion of Physician Assistant (PA) or Nurse Practitioner (NP) accredited program and ability to obtain an active, unrestricted Louisiana license (additional state licenses a plus).
- Experience and training in family medicine and the ability to treat patients from infancy to 64 years.
- Availability: 1-2 days a week - Saturday availability preferred.
- Proficiency in and experience using electronic medical records (EMRs).
- Maintains current CPR certification (BLS).
- Able to meet Nest Health credentialing requirements.
- Malpractice profile meets Nest Health risk guidelines.
- Experience serving underserved populations with complex social needs preferred.
- Experience working as a member of an interdisciplinary care team preferred.
- Experience in a startup environment is a plus.

Travel & Physical Requirements
- This is a New Orleans-based role.
- Must have a driver’s license, auto insurance, a car (mileage is reimbursed), and the ability to drive to the homes of Nest families and carry portable healthcare supplies and tools.
- Must be able to lift up to 25 pounds at times.
