Applewood Centers is a private, nonprofit agency providing mental health services, social services and education to children, adolescents and families living in Cuyahoga and Lorain counties. Dedicated to addressing a variety of behavioral, emotional and learning problems, we provide a continuum of care from outpatient counseling to residential treatment.
Position Description: The Clinical Case Manager will coordinate with the Community Based Intervention Center Day Reporting (CBIC) court staff to identify the youth in need of clinical case management or brief solution focused interventions enrolled in the program. In order to determine needs, the Applewood clinician will complete a brief assessment. Clinical decisions and interventions will be driven by the needs identified in the assessment. The clinician will develop case management services to meet the needs of the youth in order for the youth to best be served by the CBIC curriculum. All interventions for services will be in coordination with the CBIC staff and for the purpose of aftercare linkages to other community-based referrals. The Clinical Case Manager will provide the support needed to effectively and safely co-facilitate group cognitive-behavioral interventions operated by the Court’s Community-Based Intervention Center.
Position Responsibilities:Clinical Case Managers work collaboratively with court administration and staff in the CBIC setting to provide them, as well as the identified clients, with consultative support. Services are to be provided primarily in the CBIC program but may also be in the community and home setting during non-program time. This position requires flexibility of schedule and ability to travel in response to the needs of families and program.
This program does not bill Medicaid, thus there is NO PRODUCTIVITY expectations. We provide individual supervision and group supervision for LPC, LSW and MFT candidates seeking independent licensure in a supportive team environment.

Requirements:
- Bachelor level licensed social workers who have diagnostic and clinical experience will be considered.
- A Master's Degree in Social Work or Counseling, an LSW, LPC or MFT is preferred.
- Prior experience in the delivery of mental health services in the school/community to children and families with complex needs is essential.
