Trinity Community at BeavercreekThe RN Care Manager assures appropriate nursing services are provided to all home health patients as specified in the patient’s plan of care as ordered by the attending physician, effectively supporting the quality of care needed to achieve expected outcomes.
Essential Functions Statement(s) - Facilitate initial evaluation visit of patient in their home (Start of Care) or, when possible, before institutional discharge. Set the foundation for services to be provided.
- Participate in the development of the Plan of Care, communicating with the attending physician, home care team members and client/family and/or caregivers as it relates to physical therapy services.
- Provide direct nursing care and services, based on the Plan of Care and in accordance with accepted nursing practice and as directed by the attending Physician.
- Provide supervision of assigned staff relative to implementation of the Plan of Care.
- Provide education to client and/or family/caregivers regarding new skills and / or activities of daily living in accordance with the Plan of Care. Support the independence of the patient by self-care or family member care.
- Provide planning for the needs of the patient after discharge, including by not limited to identifying and obtaining necessary equipment throughout care as well as arranging for continued delivery of supplies following discharge from care.
- Make follow-up calls after discharge per policies, procedures and guidelines.
- Ensure referrals to other services (PT, OT, ST, SW, hospice or palliative care) are communicated to all necessary individuals, including other Home Health staff and patient’s physician, and ensure these referrals are followed through with and necessary care is in place.
- Participate in meetings and care conferences as needed.
- Document homebound status, medical necessity, skilled interventions, nursing services provided and all other documentation required according to policies and procedures.
- Complete Home Health Advanced Beneficiary Notice and expedited review notices as required.
- Maintain Personal Care Specialists’ assignments, effectively communicating all changes.
- Generate orders, document information in EMR and provide all necessary information to support billing processes within specified timeframes.
- Participate and comply will all deadlines for annual education, employee health screenings, and meeting attendance, as required.
- Comply with all home health policies, procedures and regulations.
- Perform all other duties as assigned.

Skills & Abilities
- Education: Must be a graduate of a two (2) year accredited nursing diploma school or have a Bachelor's Degree in nursing.: Required; Successful completion of a state-approved pre-licensure nursing education program; Passing of National Council Licensure Examination-Registered Nurse (NCLEX-RN); Active RN license conferred by the State Board of Nursing; Must obtain and maintain mandatory, state and federal requirements and certifications for practice or occupation; Must be CPR certified
- Experience: One (1) previous experience, preferably in a related field.
- Computer Skills: Must be able to proficiently use a computer, the Internet and basic office equipment; Familiarity with Microsoft Office Software.
- Other Requirements: Must have a valid driver’s license. Must be able to effectively perform essential duties while supporting UCH’s mission, vision and values, in a professional and ethical manner.
