We are a large practice with 25 providers that serve Skilled Nursing Facilities, Assisted Living Facilities and our homebound patients in the comfort of their home in 14 counties. Our medical practice is currently looking for an outstanding Patient Advocate Specialists to join our team in Orlando and surrounding areas as the Facility Lead Patient Advocate Specialist. The Facility Lead Patient Advocate Specialist works closely with the Patient Advocate Manager to establish clear staff expectations and ensure they are met. Providing compassionate communication with patients and caregivers, coupled with strong documentation of all activities, forms the core of this position. The responsibilities of the Facility Lead PAS include but are not limited to:
- Maintain up to date knowledge of CMS guidelines for Chronic Care Management
- Train new hires at assigned facility.
- Weekly reporting of assigned staff productivity.
- Ensure Facility and CCIQ Census is properly maintained.
- Enroll patients into the CCM and RPM insurance benefits as appropriate.
- Communicate with provider(s) that round at facilities.
- Communicate with nurses and staff at the facility.
- Review charts for completion, and obtain needed updates/documentation.
- Check in with patients for Wellness Checks.
- Complete monthly medication reconciliation for assigned patients
Skills:
- Must be certified (CMA, RMA, EMT, LPN, CNA or similar)
- Medical terminology a plus
- Medication terminology a plus
- CCM experience a plus
- Some travel required from and to assigned facilities.
