Summit Medical Consultants is recruiting for a Case Manager that will support our practice with clinical administrative assistance. This is a full-time hourly position, with a full benefits package.
Summit Medical Consultants is a rapidly growing Physician Practice. Founded in 2015 with 3 employees, we have expanded to approximately 75 employees and are continuing to grow. If we were a hospital in Colorado, we would rank 9th by number of admissions per year. Our mission is to provide patients and their families with robust clinical services throughout their Acute Inpatient Rehab, Skilled Nursing, Long Term Care, and Assisted and Independent Living stays. We coordinate with the Hospitalists and Specialist physicians and surgeons, Physical, Occupational, and Speech Therapies, Case Management, and Psychiatry at the facility level to provide compassionate and coordinated care.
Job Responsibilities:
- Assessing patient’s physical and mental wellness, needs, preferences, and abilities to develop tailored care plans.
- Working with clients, family, and friend support networks and health care professionals to implement care plans.
- Recording clients’ progress including charting referrals, home visits, and other notable interactions.
- Evaluating clients’ progress periodically with the clinical provider team and making adjustments as needed to improve outcomes.
- Coordinating discharge planning for skilled nursing patients.
- Managing and building relationships with Home Health and Hospice partners.
- Assisting the Practice Management team in tracking, monitoring, and intervening in ACO/ISNP and Optum patient metrics.
- Assisting provider teams in Annual Wellness Visits within our Long Term Care and Skilled nursing facilities.
- Tracking and implementing improvement plans to decrease return to hospitalizations and improve overall health and wellness for our patients.
- Utilizing critical thinking and problem-solving skills to determine the best plan of care after assessing patient’s chart, reviewing with clinical providers, and implementing interventions.
- Managing and communicating barriers to the Practice Management team.
- Participating in the transitional care management program by scheduling, assisting, and completing TCM calls and visits.
- Assisting with TeleMedicine visits as assigned by the Practice Manager.
- Maintaining positive professional relationships with company vendors.
- Building and maintaining new relationships with facilities and Home health vendors.
- Training care coordinators.
- Performing administrative duties as assigned.
- Overseeing the care coordination team.
- Delegating appropriate assignments to care coordinator teams (MA/LPN).
- Participating in the admin on-call rotation with other admin personnel.
- Managing patient caseload for the entire practice with support from Care Coordinators and Triage Manager.

Job Qualifications:
- LPN or RN active license.
- Minimum of 1 year Case Management or Post-acute care experience.
- Experience in the use of Electronic Medical Records.
- Preferred experience in ACO and/or value-based care models.
- Strong written and verbal communication skills.
- Experience with Google Suites and Smart Sheets preferred.

Pay Range:
- Salaried- $65,000 - $70,000.
