Sage Specialty Hospital in Denham Springs has Full-Time and PRN Night positions available for Licensed Practical Nurse (LPN) with LTAC experience.

Responsibilities:
- Provides nursing and supportive care in accordance with Interdisciplinary Plan of Care, under the direction of the RN.
- Communicates with the attending physicians, other hospice team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
- Documents all services provided and submits on appropriate forms.
- Demonstrates knowledge of Medicare/Medicaid regulations for hospice benefits.
- Reports changes in patient condition and needs to RN.
Qualifications:
- At least 1 year experience with either LTAC or Surgical unit.
- Must be able to work a flexible schedule including 12-hour shifts.
- Have dependable transportation, driver’s license, and car insurance.
