Our offices are located in Rocky Hill and North Haven.
Minimum Qualifications:
- Current and active CT Licensed Practical Nurse License in good standing
- Graduate of an approved LPN / Licensed Practical Nurse program (per state requirements)
- 1 year of clinical nursing experience
- Must have own transportation and be reliable
- Flexible availability during the afternoon, evenings, and weekends.
- Good organization and communication skills
- Ability to follow and carry out instructions
- Experience with electronic documentation/point of care systems a plus
- Previous home care experience a plus
- Language: Spanish (required)
- Proof of satisfactory completion of a medication course a plus. (not required)
Summary:
Under the direction of a Registered Nurse (RN), the LPN visit nurse is responsible for providing routine skilled nursing care in the home, consistent with accepted nursing practice and agency policies. The LPN is responsible for keeping the RN, Supervisor of Clinical Services, and the attending physician informed on each patient’s status and progress. The LPN operates within the scope of practice defined by the State Nurse Practice Act. The LPN contributes to nursing assessments and care planning, provides direct patient care, and supervises patient care provided by unlicensed staff.
Duties and Responsibilities:
- The LPN assists the RN in carrying out the patient’s plan of care.
- Observes conditions and reports changes in condition or family situation to RN
- Consults and seeks guidance from the RN as necessary
- Participates in the development, implementation, and continuing evaluation of the patient care plan.
- Participates in case conferences and staff consultation
- Participates in ongoing patient/family teaching
- Prepares, administers, and records medications per Agency policies
- Successful LPN visit nurses will need to have strong interpersonal and organizational skills, and be able to handle multiple tasks.
