Responsibilities
- Utilizes wound dashboards to identify high risk and high visit utilization patients and focus wound patient populations for consultation.
- Utilizes wound dashboards to identify new wound patients and works with care center leadership to assure the plan of care is individualized and meets standards of care.
- Provides real time virtual consultation utilizing the TEAMs platform in conjunction with a clinician visit.
- Provides traditional consultative services via wound photo and chart review and conversation with the clinician/CM.
- Develops evidence-based recommendations for care to drive best patient outcomes.
- Monitors and analyzes data for high risk wound patients, patient outcomes, and goal achievement.
- Ensures programs and initiatives are in place to achieve outstanding clinical outcomes and operational efficiency.
- Promotes nationally recognized standards of practice in wound care.
- Ensures overall growth of wound care services in the assigned service area by assisting regional, POD and care center leadership in management of wound care patients, supplies, appropriate product use, application of evidence-based practices and standards of care and appropriate visit utilization.
- Provides leadership, clinical and operational support for staff and serves as a resource for care center /POD based wound care specialists and champions.
- Assists with implementation of clinical initiatives for the company.
- Serves as part of a national group of wound care leaders to share best practices and to drive consistency and standardization of wound care practices and operational efficiencies.
- Supports the company's performance improvement plans.
- Utilizes wound dashboards to ensure basic and advance wound care services are being delivered in accordance with company policy and evidence based clinical standards.
- Provides guidance for appropriate wound patient referrals for both the business development team and care center clinical decision makers.
- Provides statistical report analysis to improve care and collaborates with regional leadership to meet quality metrics.
- Collaborates with the Corporate Clinical Group and other regional leaders to drive consistency and standardization of best practices.
- Obtains and analyzes data related to wound care services.
- Identifies trends/problems and discusses with regional, POD, care center leadership.
- Develops clinicians through guidance, coaching and mentoring.
- Serves as a resource to Regional, POD and care center leadership.
- Performs other duties as assigned.
Qualifications
Required-
- Current, unencumbered state license to practice as a Registered Nurse.
- Nurse Licensure Compact (NLC) documents the following: The mutual recognition model of nurse licensure allows a nurse to have one license (in his or her state of residency) and to practice in other states (both physical and electronic), subject to each state's practice law and regulation. Nurses must legally reside in a NLC state to be eligible to have a multi-state license.
- Board Certified with the Wound, Ostomy, Continence Nurses Society, the American Board of Wound Management or has completed an advanced practice nurse program (WOCN, CWS, MSN, APRN).
- Three (3+) years' experience working as a Registered Nurse performing wound care in a clinical setting, with one (1+) year of home health care experience preferred.
Preferred-
- Working knowledge of the HHC industry regarding wound care including Medicare/Medicaid, commercial insurers, PDGM, VBP, and STARS.
- Working knowledge of remote consultation using technology platforms.
- Effective communication skills.
