We are looking for Nurse Practitioners (NP) and Physician Assistants (PA) to join our team in conducting patient-centered care in patients' homes under Medicare.
The Nurse Practitioner or Physician Assistant will be accountable for caring for patients, maintaining accurate and current patient records, and scheduling initial, urgent, and follow-up appointments as required.
Competencies:
- Perform initial patient screenings, physical examinations, and create assessment plans.
- Address urgent care needs of the patients as needed.
- Order, interpret, and analyze diagnostic tests and determine the need for follow-up appointments and further treatment options.
- Prescribe and manage medications and treatments as appropriate, including but not limited to psychopharmacological agents.
- Deliver palliative care, and when needed, end-of-life care to patients and families.
- Deliver quality care while maintaining all company metrics pertaining to appropriate documentation, coding, HEDIS measures, engagement of patients, and managing medical expenses.
Qualifications:
- Experience in family practice, geriatrics, hospice care, palliative care, hospitalist, ICU, SICU, or ER.
- Master's Degree.
- Valid Physician Assistant or NP license and Board Certification.
- NPI number.
- BLS/ACLS certification.
- Valid driver's license, auto insurance, and willingness to drive to patients' homes/patient locations.
- Salary: $103,000 - $137,000 plus DOE, excellent benefits package including 4 weeks PTO and more.
