Psych Nurse Practitioner (Outpatient) Community Health – South Shore
Great opportunity for a psychiatric nurse practitioner to join a growing team working in a community health environment providing care to the under-served population.
The practitioner will work with a team of psychiatrists, Primary Care Physicians, and Social Workers to provide comprehensive and appropriate continuum of care.
Duties include:
- Performing psychiatric diagnostic assessments for all referred patients.
- Prescribing medications and providing medication management.
- Assessing, planning, documenting, and evaluating patient needs and treatment.
- Providing consultation for primary care providers regarding patient treatment.
- Providing intervention and referral for patients when appropriate.
- Providing counseling, crisis intervention, and advocacy to all patients and their families, taking into account their unique cultural backgrounds.
- Psychoeducation around such issues as high-risk behavior, mental health, addictions, and domestic violence.
