Job Summary
The Heart Failure Nurse Navigator is a specialized nursing role focused on providing comprehensive care and support to patients with heart failure. Job Requirements
- Education
- Required: Graduate of an approved nursing program
- Preferred: Bachelor’s or Master’s degree in nursing, business or health related field
- Experience
- Required: Three years clinical experience in cardiology or heart failure management, preferably in an acute care or specialized outpatient setting
- Skills
- Excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare professionals.
- Proficiency in using electronic health records and other healthcare software systems.
- Ability to work independently, prioritize tasks, and manage a caseload of patients effectively.
- Demonstrates commitment to professional development, staying current with research and advancements in heart failure care.
- Licensure/Certification/Registration
Required: MO RN License
- Preferred: Specialized certification in cardiac nursing, heart failure management, or a related field (e.g., Certified Heart Failure Nurse)
