POSITION SUMMARY:
The Licensed Practical Nurse implements the interdisciplinary resident care plan by providing excellent direct care and oversight and direction to CNA team members.
PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:
- Reviews the status of each resident daily or more frequently if required.
- Identifies and reports change in resident condition.
- Knows resident choices regarding care and assures they are honored.
- Leads the CNA team in following comprehensive resident care plans.
- Evaluates if the team has effectively met resident needs.
- Delegates the completion of the care plan work to team members according to their licensure experience and expertise.
- Supervises the delivery of care to assure it is being delivered according to the facility policies and standards of care.
- Participates in care delivery by providing medications and treatments and other nursing tasks as assigned.
- Participates in the interdisciplinary care planning process.
- Evaluates the outcomes of the care delivery.
- Adjusts the care plan based on outcomes.
- Counsels and coaches employees who exhibit performance discrepancies.
- Documents care and files reports as required.
- Other duties and responsibilities that may become necessary or as directed by the unit manager or DNS.
- Any additional tasks or responsibilities as assigned by the Assistant Director of Nursing, Director of Nursing, or Health Center Administrator.
QUALIFICATIONS:

Educational Requirements and Experience:
- Must possess a current, unencumbered, active license to practice as an LPN in Massachusetts - required
- Current CPR certification - required
- Knowledge of standard nursing practice, state regulations, and the Nurse Practice Act.

The Registered Nurse leads the team in providing excellent holistic, resident-centered clinical care and habilitative support in activities of daily living, using servant leadership. Leads in accordance with the current federal and state regulations and JCAHO standards.

- Delegates the completion of the care plan work to team members according to their licensure experience and expertise using the guidelines of the Nurse Practice Act.
- Supervises the delivery of care to assure it is being delivered according to the facility policies and standards of care.
- Participates in care delivery.
- Evaluates the outcomes of the care delivery.
- Adjusts the care plan based on outcomes.
- Counsels and coaches employees who exhibit performance discrepancies.
- Accurately and comprehensively documents care and files reports electronically and on paper as required.
- Other duties and responsibilities that may become necessary or as directed by the unit manager or DNS.
QUALIFICATIONS:
