POSITION SUMMARY:
The Registered Nurse, Baylor plans and provides individualized nursing care and comprehensive services to hospice patients. The Registered Nurse, Baylor carries out expected duties as assigned by clinical leadership based on patient need.
The Registered Nurse, Baylor is expected to be available to see patients beginning at 5:00 PM on Friday and continuing through 8:30 AM on Monday and will be paid for 40 hours per week.
PRINCIPLE DUTIES/ESSENTIAL FUNCTIONS:
- Assesses patient’s health problems and needs and develops plan of care.
- Provides direct skilled nursing care as per physician orders.
- Observes patients, charting and reporting changes in patient’s conditions, such as adverse reactions to medications or treatment and taking any necessary action.
- Document medical records and prepare other reports as needed.
- Educates patients and families in self-care and health management.
- Collaborates with the physician and other home health care providers, within and outside agency to enhance patient care.
- Provides case management when appropriate.
- Attending case conference as requested.
- Adheres to policies and procedures.
- Participates in agency quality management and safety activities.
Weekend Baylor Expectations and Guidelines:
- The Registered Nurse, Baylor is expected to be available to see patients beginning at 5:00 PM on Friday and continuing through 8:30 AM on Monday and will be paid for 40 hours per week.
- The expectation is that this role will maintain the highest of attendance standards; staff selected for the Weekend Baylor should NOT have any unexcused callouts within the past 6 months.
- Overlook's Attendance and Tardiness Policy will be strictly enforced with zero tolerance and no excuses.
- Staff selected for the Baylor Plan must be in good standing, with no disciplinary action occurring within the past 6 months.
QUALIFICATIONS:
- Current license to practice as a registered nurse in the state of Massachusetts.
- Current CPR required.
- Three years nursing experience, preferably in hospice.
- Excellent nursing assessment and problem-solving skills.
- Excellent interpersonal communication skills, both oral and written.
- Exceptional customer service.
- Strong clinical assessment, critical thinking, problem-solving and organizational skills.
- Ability to utilize independent judgment and to follow through.
- Current State of Residence driver's license and reliable transportation.
- Computer proficiency.
