Currently, we are looking for a Registered Nurse Case Manager who would like to combine their love for people and excellent clinical and leadership skills with the opportunity to advance your career.
Our RN Case Managers plan, organize, and provide exemplary nursing care to our patients. They build from the resources of the community to plan and direct services to meet the needs of the individual and families within their homes and communities.
Responsibilities
- Complete initial, comprehensive, and ongoing assessments of patients and families to determine patient needs.
- Develop care plans that establish goals, based on nursing diagnosis, and incorporating palliative nursing actions, including the patient and the family in the planning process.
- Communicate with community health-related persons to coordinate care plans.
- Work in cooperation with families/caregivers and interdisciplinary groups to meet the emotional needs of the patient and family/caregiver.
Requirements
- Must have active RN license and CPR certification.
- Must maintain a valid driver’s license, active auto insurance meeting state minimum requirements, and an operational vehicle.
$80,000 - $90,000 a year
