Position: Care Consultant

- About the Role: We are seeking an experienced and motivated Care Consultant to join our Home Care Packages Team. The role involves providing crucial support and advice to Home Care Packages clients, empowering them to maintain independence and well-being in their own homes.
Key Responsibilities:
- Building positive, long-term relationships with clients and allied health professionals
- Developing and maintaining Care Plans for clients
- Expectation management of clients and stakeholders
- Identifying and realizing business growth opportunities
- Ensuring the safety of support workers
- Collaborating effectively with service coordination staff
Skills and Experience Required:
- Tertiary qualification in Welfare, Human Services, Social Work, Nursing, or Allied Health
- Experience in Disability and Aged Care sectors
- Previous experience with Home Care Packages clients (desirable)
- Familiarity with Carelink +
- Strong communication and reporting skills
- Ability to work autonomously and in a team environment
- Current NDIS Clearance, WWCC, Statutory Declaration, and Driver's License
- Access to own vehicle
