Key Responsibilities
- Evaluate, treat, and direct treatment for patients for whom physical therapy is medically necessary.
- Educate all patients continually on their condition to ensure their commitment to treatment.
- Maintain ongoing documentation in which goals, progress toward functional goals, and medical necessity are clearly stated.
- Complete all required documentation (Initial Evaluations, Daily Treatment Notes, Plans of Care, Discharge Summaries, Insurance Forms, etc.) thoroughly and on a same-day basis in order to meet facility, physician and payor deadlines.
- Maintain open and respectful communication with co-workers, physicians and staff.
- Able to confidently and knowledgeably communicate with patients, developing a strong patient rapport throughout the process.
- Attend facility in-services and participate in courses necessary to maintain required CEUs.
- Act as a role model and as a resource for peers.
- Research unfamiliar diagnoses, procedures, and requests as opportunities to provide needed learning experience.
- Utilize time efficiently and effectively to meet productivity requirements of the facility.
- Understand appropriate state Practice Act and adhere to the laws which govern how Physical Therapists may practice and whom Physical Therapists may supervise.
- Assist with general facility and equipment maintenance duties to ensure that clinic remains safe, clean and organized.

OSG Mentorship Program Highlights
This role offers the chance to be mentored by experienced professionals, providing valuable insights and growth opportunities. You will attend educational in-services, develop relationships with current Physical Therapists, and participate in weekly meetings with your mentor(s).
- Educational and Clinical Growth: Provides new physical therapists with supervision and support, fostering critical thinking and professional development.
- Initial Onboarding: Includes personalized goal setting, shadowing senior therapists, restricted schedule, and regular meetings with mentors and the Rehab Director for the first 1-3 months.
- Ongoing Mentorship: Continues with monthly mentor meetings, quarterly Rehab Director meetings, optional senior therapist consultations, and opportunities for physician, surgical shadowing as well as monthly in-services.
Required Credentials
- Graduate of a CAPTA-accredited program in Physical Therapy
- Current CT Physical Therapy License required
- Experience preferred but new grads are encouraged to apply
