Salary Range: $110,000.00 to $200,000.00 a year with quarterly bonus opportunity.
By adhering to Connecticut State Law, pay ranges are posted. The pay rate will vary based on various factors including but not limited to experience, skills, knowledge of position and comparison to others who are already in this role within the company.

An Internal Medical Group has an opportunity for an Advanced Practice Registered Nurse (APRN) or a Physician Assistant (PA) to work in Assisted Living Facilities in the Mystic area. This is an excellent opportunity for those who enjoy a flexible work schedule, working autonomously, and excel in a pay-for-performance environment.
The Nurse Practitioner or Physician Assistant will travel to Assisted Living Facilities contracted with PACT, LLC.
Responsibilities
- Educate patients on the benefits of wellness visits vs. physicals.
- Create care plans for patients.
- Utilize EHR, EPIC to update patient charts.
- Engage patients and their families or caregivers in understanding, setting, and monitoring self-management care plans, and document individualized care plans and care coordination in the patient chart.
- Analyze patients' medical history, medication allergies, physical condition, and examination to determine the best care.
- Diagnose medical conditions.
- Prescribe pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
- Direct and coordinate the patient care activities of nursing and support staff as required.
- Follow established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
Education and Experience
For APRN:
- The Advanced Practice Registered Nurse is masters or doctorate prepared.
- Certified nationally as a Nurse Practitioner through AANP or ANCC.
- The Nurse Practitioner must have possession of a valid Connecticut license as a Registered Nurse and an Advanced Practice Registered Nurse, and be certified as a Nurse Practitioner by an approved certifying body.
- DEA and CT controlled substance license.
- Experience working with the elderly patient population.
- Experience working with EHR.
For PA:
- State License.
- Any years of experience.
- Experience working with EHR.
