Native American Connections (NAC) is looking for a dedicated medical provider to serve in the position of Family Nurse Practitioner. The ideal candidate for the position must be aligned with the NAC’s environment and culture and focus on fulfilling the NAC’s mission, vision, and values.
The Family Nurse Practitioner is a key member of the Integrated Health team, engaged in providing medical services and helping to define the medical direction of the organization. This hybrid position provides primary care medical services in the office and via mobile unit to adults and children to ensure the delivery of affordable quality healthcare services. The Family Nurse Practitioner works collaboratively with the Integrated Health staff to ensure clients transition seamlessly between medical and behavioral health services.
The successful candidate for this position must have:
A minimum of two years’ experience providing medical services. Experience in public sector healthcare or non-profit community health, with knowledge of Medicaid populations; experience working with Native Americans preferred.
A minimum of a master’s degree in nursing and AZ Board of Nursing qualification as a Family Nurse Practitioner with an active and unrestricted license.
Experience and Competencies:
- Works collaboratively with team to provide primary health care to clients including: obtaining histories, performing physical examinations, diagnosing, treating and monitoring acute and chronic diseases, immunizations, ordering and interpreting diagnostic studies, prescribing medications, and providing other treatments as needed.
- Performs CLIA waived testing.
- Determines need for confirmatory test and prepares specimens for send out to referral facilities by preparing appropriate requisition for the reference laboratory both manually and by computer entry.
- Performs appropriate quality control procedures for testing procedures and documents in accordance with departmental policies, maintaining appropriate records and files.
- Collaborates with Behavioral Health professionals for care coordination as needed.
- Educates clients to promote positive health behaviors and disease prevention.
- Takes all steps necessary to ensure accuracy of patient data, test documentation, and recordkeeping, including updating the electronic medical record with testing orders and reporting results to state, federal and other reporting agencies.
- Assists with delivery of direct care/patient services within NAC residential and/or housing facilities via mobile clinic.
- Ensures a safe working environment by adhering to the standards set by Occupational Safety and Health Administration.
- Complies fully with all patient and data confidentiality policies and regulations, including, but not limited to HIPAA.
- Provides feedback regarding the medical direction of the organization.
- Keeps abreast of emerging models in health care delivery; identifies and defines new and innovative strategies to achieve business goals and objectives.
- Identifies opportunities to collaborate and develop clinical integration opportunities with health delivery systems to achieve affordable outcomes.
- Completion of disability certifications.
- Conducting telehealth appointments as necessary.
Salary Range: $120,000 to $140,000
