In response to the humanitarian crisis at America’s ports of entry, Devereux provides short-term shelter care services for unaccompanied children who have come to the United States without a parent or legal guardian. Devereux provides extremely high-quality, trauma-informed residential and educational services for unaccompanied children, with support for their social and emotional health.
Schedule:
9:00am to 5:00pm Monday-Friday. On-site (NO remote)
Primary Responsibilities Include:
- Develop master treatment plans for children, families, and necessary providers; monitor plans to ensure progress and compliance.
- Attend medication appointments or psychiatric evaluations as needed.
- Serve as the liaison for resources and ensure all referrals are made for medically necessary treatment.
- Initiate, develop, and maintain collaborative relationships with children, their family members, and significant support systems.
- Facilitate meetings and compile documentation from all internal and external providers.
- Serve as liaison for system navigation in obtaining community services and entitlements, securing appointments with outside services and agencies, including identification of funding sources and linkages to other agencies/services.
- Complete referrals, assessments, treatment plans, and required documents.
- Demonstrate knowledge and skill necessary to obtain appropriate care, treatment, and services for the behavioral health needs of the children.
- Serve as primary liaison in regard to all levels of care, discharge planning, and any other area that the family needs support.
Qualifications:
- Bachelor’s degree (behavioral science, human service, or social service field preferred).
- Ability to work in conjunction with clinicians and other team members.
- Must be empathetic and have a team approach to care for individuals.
- Excellent written and verbal communication skills.
- Fully immunized/vaccinated (or have a medical/religious exemption) for Varicella, MMR, TdAp, Influenza prior to starting.
- Must be 21 years of age or older (per licensing standards).
- Valid Connecticut Driver's License.
