POSITION: Early Intervention Coordinator/Case Manager

SALARY: $48,000-$50,000, Dependent on qualifications
HOURS: Full-Time, Non-Exempt
Description:
This position has two components, based on funding sources. The first component will provide early intervention services and placement prevention services to American Indian families by connecting them to needed resources, assisting with cultural support and connections, and addressing emergency needs. The second component involves planning and development for the distribution of Early Intervention Funds for American Indian families who are in the early stages of instability and would benefit from access to resources. The Early Intervention Coordinator will work with internal and external partners to identify eligible families needing these funds and manage the distribution and tracking of these funds.
Qualifications:
- Bachelor’s degree, preferably in Social Work, or related field.
- Experience in administering/managing services.
- Prefer experience in case management.
- Possess excellent communication and written skills.
- Experience working in the Native American community. Possess knowledge and understanding of Native American history, culture, community institutions, and historical trauma.
- Knowledge of community social service resources.
- No current drug or alcohol problems.
- Ability to successfully pass a Criminal Background check.
- Valid Driver’s License, insurance, and access to vehicle.
Duties and Responsibilities:
- Conduct intake with new clients to assess needs.
- Refer clients for chemical health programs, CD assessments, parenting programs, culturally based services, educational services, employment, and any other needed services.
- Assist families in connecting to monthly family orientated cultural groups and activities.
- Assist in planning of quarterly cultural activities.
- Manage Early Intervention Funds budget.
- Coordinate team meetings, both online and in-person, with stakeholders in the community, using technology when needed.
- Participate in staff meetings.
- Work with staff and partners to collect information and compile for program reporting.
- Work with Program Director to develop policies, protocols, and forms for Early Intervention Fund.
- Develop partnerships and provide information about Early Intervention Fund to staff, programs, and agencies that work with American Indian families; develop MOU’s with partners to participate in fund distribution/monitoring program.
- Provide training to participating staff/partners on policies and protocols for accessing the Early Intervention funds for clients/participants and tracking results.
- Manage fund distribution including interface with partner agency, families, and the Minneapolis American Indian Center Finance Department to issue checks/payments.
- Maintain careful records that comply with accounting requirements and state requirements.
- Work with partners to implement monitoring/evaluation of results of receiving funds on family stability.
- Compile records to complete quarterly reporting to the state.
- Assist with case staffing and consultations as needed.
- Other duties as assigned by Supervisor.
