Description
Position: Bilingual Support Specialist
Support Specialists are the first Monogram Health voice the patient hears. Once a patient is enrolled, the Support Specialist is responsible for scheduling in-home care management visits and coordinating with physician practices to schedule office visits and exchange notes. The Support Specialist will often field patient calls, assist the patient in troubleshooting biometric devices, or route them to other members of the care team for assistance.
Roles and Responsibilities
- Conduct outbound calls to existing patients to gather needed information and schedule in-home care management visits, telehealth meetings, physician office visits, dialysis center tours, group support sessions, and other appointments as needed
- Contact physician offices and other providers by phone, fax, and email to arrange patient appointments or coordinate the sharing of clinical information
- Field inbound calls
- Enter data as needed
- Assist District Managers in the creation, distribution, and tracking of equipment and materials
Position Requirements
- High School diploma or GED is required.
- Must be 18 years of age or older.
- Must be fluent in English and Spanish.
- Excellent oral, written, and comprehension skills.
- Experience in customer service, sales, and healthcare.
- Basic computer skills, as well as experience with Microsoft Word and Outlook.
- Positive attitude and a desire to help our patients with their kidney disease to improve their quality of life.
- Must pass background check and drug test.
