Hiring Range: Starts at $33/hour. Higher starting wage depending on experience, education, certifications, etc.

Your Impact:
- Perform medical patient assessments and assist in developing individualized care plans
- Advise and administer medications, treatments, and therapies, as prescribed
- Monitor patient progress and provide ongoing evaluations and updates to the care team
- Educate patients and unlicensed direct care employees on health management and home care procedures
- Coordinate care with other healthcare professionals and community resources
- Maintain accurate and timely documentation of patient care and progress
- Perform other duties as assigned, contributing to the success of the program and the overall mission of the organization
What you'll need:
- Valid RN license in Minnesota
- 2 years of nursing experience or equivalent, preferably in home health or community health settings.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Department of Human Services background clearance
- Valid driver's license, reliable vehicle, acceptable driving record
- Ability to effectively communicate in English, both written and verbally
