Full Time Nurse Practitioner - Stroke Program - Modesto area

Experience needed: MSN, Nurse Practitioner, Must be CA licensed RN/NP. NP with stroke experience preferred; however, the team is willing to consider a new grad NP with a strong ICU, neuro, and ED experience as a RN. DEA, BLS, ACLS.
Responsibilities:
- Stroke Program Nurse Navigator (Nurse Practitioner) provides expert nursing consultation and practice oversight, functioning as a liaison between medical staff, nursing staff, family members, and support services.
- Coordinates multidisciplinary care based on evidence-based practice. Incorporates the patient experience in all aspects of patient care.
- You are responsible for providing stroke education, delivery and oversight of patient care, including post-discharge follow-up.
- Works in collaboration with Stroke Program key stakeholders to meet stroke program certification requirements, including review of data and performance improvement projects.
- Reports to the Stroke Program Coordinator.
