This is a busy, fast paced Home Health rehabilitation team which provides Physical Therapy, Occupational Therapy, and Speech Language Pathology services to the community.

The Requirements:
- Degree from an accredited school approved by CAPTE
- Current state licensure or eligibility in Physical Therapy in Maryland
- Has one (1) year Home Health experience, but would consider candidates with experience in the inpatient or skilled settings as well
- Basic Life Support through the American Heart Association
- Will display strong skills in flexibility, time management, leadership, and communication
- Focuses on the patient and providing the utmost in customer service excellence
- Experience with documentation utilizing EMR
The responsibilities:
- Deliver quality outcomes driven physical therapy services
- Document with web-based software and documentation system (EMR)
- Provide accurate and timely documentation for current patient caseload and oversee compliance of department.
- Conduct patient and family education and meet all clinical, department, and regulatory expectations
- Participate in a positive work environment to promote teamwork and communication
