Nutritionist II - Great Beginnings/North Vallejo
Who we are: La Clínica de La Raza is a community-based health center committed to providing culturally appropriate, high-quality, and accessible health care to the diverse communities of the San Francisco Bay Area. We have spent over 40 years advocating for and creating a health home for the many that have been denied access to care. As health care activists, we are dedicated to making sure individuals who do not have health coverage get the same level of quality healthcare as those who have it.
From our genesis as a community health center in 1971, we understood that we cannot heal our communities without also addressing the economic and social factors that affect health. Many times, this requires that we go outside and provide services in other settings to build the bridge that links individuals to our health center for preventive and primary care. Over 90,000 individuals come to La Clínica because it is a welcoming place that addresses the whole person, coordinating and connecting them to a broad network of services to improve and maintain their health and well-being. While we are still known for our activism and spirit of social justice, we are also proud to have grown into a sophisticated provider of primary health care services with 35 sites across Alameda, Contra Costa and Solano counties.
About the Position: Under general supervision, the Nutritionist II will conduct nutrition screening, assessment, and counseling for a diverse population on various nutrition-related diseases such as hypertension, high cholesterol, obesity, and diabetes. The role also involves implementing nutrition care plans for patients with diabetes, pre-diabetes, gestational diabetes, and other high-risk pregnancies, hypertension, obesity, and hyperlipidemia. Additionally, the Nutritionist II will assist in curriculum development and implementation of educational materials, teaching classes, and quality improvement work.
Major Areas of Responsibility include but are not limited to:
- Conduct nutrition screening, assessment, and counseling, and implement nutrition care plans for low and high-risk patients.
- Provide individual nutrition and health counseling for those with overweight, diabetes, hypertension, and dyslipidemia.
- Maintain documentation by charting SOAP notes in progress notes.
- Develop and participate in group nutrition education programs and medical group visits.
- Collect and review educational materials; assist in the development and evaluation of culturally relevant nutrition education materials.
- Assist in data entry and retrieval and other quality improvement efforts for the department.
- Promote site-based employee wellness efforts.
- Provide in-service nutrition/health education to staff and community partners.
- Participate in meetings related to nutrition, clinical health education, and wellness as required/assigned.
- Other duties as assigned by supervisor.
- Requires completion of Dietetic and Food Administration Program from an accredited university; RD or RD eligible, plus one year experience in a community health care setting. Masters in Nutrition/Dietetics preferred.
- Must hold a current BLS (Basic Life Support) Certificate, with a 30-day grace period to obtain it from the date of hire.
- Knowledge of contemporary principles and practices of nutrition, nutritional value of foods, and culturally appropriate meal planning and preparation.
- Knowledge and experience with Motivational Interviewing, Stages of Change, Division of Responsibilities.
- Interpretation of anthropometric, biochemical, clinical, and dietary information, and provision of culturally appropriate dietary advice.
- Knowledge of food storage and preparation principles; interpretation and implementation of special diets.
- Ability to develop, implement, and evaluate nutrition education activities and coordinate them with other departmental programs.
