Position in this function provides skilled nursing care in a home care environment. Assess, plan, and document the homecare nursing process; educate and provide instruction to patients and their family. Assistance in the management of home care site operations.
This is a Hybrid position that blends some travel and remote work in the South Florida geographic area.
Supervisory Responsibility:
- Home health administrator responsibilities as assigned
- Assisting in the management of the clinical and administrative staffing in the local home care site
- Special project management as assigned
- Additional responsibilities as assigned
Primary Responsibilities:
- Positively represent the Company to both internal and external customers; always maintain a professional and courteous manner

- Adhere to Company privacy guidelines; ensure compliance with local, state, and federal regulations
- Consistently model the organization’s philosophy of customer service by demonstrating responsibility for improvement of operational efficiency, empowering employees to address service issues with knowledge and support and to communicate the expectations of dedication to both internal and external customers
- Assist with recruitment, hiring, orientation, and evaluation of homecare site personnel
- Actively participate in budgetary management of the homecare site(s)
- Monitor staff compliance with ongoing education, training programs, and competency evaluations; document results and implement corrective actions as needed
- Maintain competency on all services provided according to policy
- Provide services according to Optum policy and the prescriber’s plan of treatment
- Individualize patient assessments based on care/services being provided and patient need
- Ensure interventions respect and encourage the patient’s ability to make choices
- Respect each patient’s rights and conduct business in an ethical manner
- Educate patients and families with a systematic approach, promoting patient understanding of treatment and services, health status, coping ability and patient/family involvement in the continuum of care or service
- Initiate the plan of treatment/care pathway based upon the analysis of information obtained and within scope of practice and certification
- Modify the plan of care, as appropriate, based on re-assessment, change in prescriber orders, the patient’s need for further care or services, and the achievement of identified goals within scope of practice and certification
- Coordinate care with the patient services centers, pharmacists, and other healthcare professionals to promote the continuum of care or service
- Accurately communicate (written/verbal) patient status information to prescribing physician in timely manner and provide necessary clinical information to Optum pharmacists and other healthcare professionals as necessary
- Complete clinical record documentation per Optum policy in a timely, complete, and accurate manner
- Provide service-related clinical support to key physicians/hospitals through personal contacts and in-services, as directed
- Participate in holiday and weekend duties as scheduled
- Must maintain a current CPR certification and obtain an annual screen and TB test and influenza immunization in accordance with Optum policy
- Must comply with random drug testing
- Ensure senior management decisions and directives are carried out
- Routine travel required
- May be asked to participate in training or act as preceptor
- Additional duties as assigned

Required Qualifications:
- Current unrestricted nursing license (RN) for state of residence
- 1+ years of obstetrical nursing experience
- 12 months of Charge Nurse Hospital Experience and/or Clinical Management/Leadership experience
- Technology knowledge to include desktop/laptop computer proficiency; use of analytical software, Microsoft Office, Outlook (internet and email). Attend in-services on applicable technology enhancements. Proven ability to maintain competency on all services provided according to policy
- Proven exceptional communication skills (written and verbal)
- Proven skills to deal with patient/family conflict resolution and grievances
- Demonstrated ability to work with individuals with disabilities

If the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years.
