POSITION TITLE: WARMLINE PATIENT NAVIGATOR
STATUS: Temporary | Full-Time WITH Benefits (1.0 FTE)
SALARY: $25 per hour ($52,000 per year)
LOCATION: This position is located in the Kaua’i Region Recovery Services (KRRS) department of Kaua’i Region Healthcare Enterprises. The KRRS department provides nursing, social work, and ancillary health services to involuntary and voluntary patients with mental health and/or substance abuse disorders (SUD).
JOB FUNCTIONS: We are actively seeking a highly motivated and compassionate individual to join our growing ‘ohana as our Warmline Patient Navigator. In this role, you will be responsible for being the primary point of contact for clients, providing field-based case management services for outpatient behavioral health patients. The incumbent will build strong relationships with clients to help them stay engaged in medical care and adhere to their medications, committed to removing barriers to care by identifying critical resources, navigating health care services and systems, and promoting client health. The Warmline Patient Navigator will work closely with the Care Team, which may include doctors, nurses, and other clinical staff to support positive client health outcomes. This position may require working evenings, weekends, and holidays and may float to and from locations within Kaua’i Region based on operational needs. The Warmline Patient Navigator will report directly to the Director of Behavioral Health Therapy, or its designee.

MINIMUM QUALIFICATION REQUIREMENTS
To qualify, you must meet all of the following requirements:
- Minimum high school degree or some college education.
- Commitment to the mission of care coordination.
- Passionate, trustworthy, and empathetic when working with clients.
- Ability to build relationships with different types of people, including clients, organization members, and health care providers.
- Good communication and interpersonal skills; ability to speak concisely to clients and Care Team members.
- Organized with confidential client material and appointment tracking.
- Flexible and adaptable in response to changing client and health care providers’ needs.
- Strong understanding of cultural competency with the target population.
- Computer literacy desirable.
