The Licensed Social Worker is responsible for discharge planning and for the development, implementation and coordination of policy and budgeting of the Social Services Department. Our Social Worker ensures that the psychosocial needs of residents in the health care center and their families are met, in accordance with current existing federal, state, and local standards as well as facility specific policies and procedures.
Responsibilities include:
- Completes required MDS assessments in accordance with federal, state, and local standards.
- Participates in monitoring behaviors and works closely with assistant director of nursing on psychotropic medication use.
- Completion of training on trauma informed care and completion of assessments.
- Supports the admission process of resident into the health care center from physicians, hospitals, and the public.
- Discharge planning, including arrangement of post-discharge services and ensuring that appropriate notifications are completed for any transfers, according to the community’s policies and procedures or as required by State or Federal regulations.

Qualifications:
- Licensed social worker (LSW or LCSW) required.
- Geriatric social work in a social service or medical setting - Long-term care experience preferred.
- Experience in admissions process.
