Schedule Expectations: 9:00am - 5:30pm - Monday-Friday
Summary: A Licensed Practical/Vocational Nurse is a licensed professional who provides advisory services to customers and caregivers on how to manage any health conditions they are experiencing as part of the Health Corner initiatives. The primary goal is to ensure the successful utilization of Health Corners. As the Health Advisor, the LPN/LVN will engage with the patient/caregiver to address health-related questions and provide educational material and information. If the Health Advisor finds it necessary, they may refer the customer to another clinical provider. As a Health Advisor, the LPN/LVN will not diagnose or prescribe therapy for the customer based on their condition, practicing within the scope of practice of the professional licensing body and/or scope defined by the company.
Job Responsibilities:
- Oversee Health Corner activities and provide both in-store and online diagnostic devices and solutions, and assist with proper use of devices to all customers.
- Administration of vaccines (both IM and SQ) under a physician protocol.
- Draw blood per physician request or under physician protocol.
- Perform gap closure activity as outlined, and escalate issues as needed.
- Effectively manage a recurring caseload of patients each month.
- Manage defined patient population to close gaps in case (both clinical and non-clinical).
- Evaluate customers’ needs and advocate on patient’s behalf to connect them to resources, services, and potential solutions related to their health condition.
- Identify and coordinate community resources for patients.
- Perform telephonic patient outreach.
- Provide patients with education on the proper use and setup of diagnostic devices (e.g., blood pressure machine, blood glucose monitors, pulse oximeter, etc.).
- Effectively communicate with agency staff, liaisons, other Care Facility staff, patients, and family/caregivers.
- Manage time effectively to deliver accurate and appropriate patient care.
- Maintain confidentiality of patient and proprietary information.
- Maintain a professional image.
- Participate and attend required in-services, trainings, and weekly meetings.
- Build and maintain a working relationship with the front end and pharmacy team to enhance patient engagement and experience.
- Assist patients in aisle by providing them education on OTC and first aid products, escorting patients to pharmacy for patient-specific questions.
- Maintain Health Corner space to brand standards provided by Walgreens and keep organized records in Health space in accordance with compliance policies.
Basic Qualifications:
- Active LVN/LPN license by State Board of Professional Regulations (must be able to remain current with licensures, certifications, and continuing education).
- At least 2 years' experience as an LPN/LVN providing nursing care. Self-directed with excellent time management skills and able to work independently as well as under the supervision of a registered nurse or another clinician.
- Must be willing to obtain NPI number before employment begins.
- Must be willing to obtain free Yellow Fever vaccine certification from CDC website.
- Must have experience with a variety of patients (including children and the elderly).
- Flexibility and the ability to work effectively within a multidisciplinary team.
- Current CPR/BLS certification, up to date on Bloodborne pathogen training.
- Ability to communicate effectively (verbal and written) with team members, patients, caregivers, physicians, and referral sources.
- Willingness to work a flexible schedule, including evening and weekend hours.
- Comfortable with digital technologies such as website forms, mobile applications, PC/tablet systems, in-store systems, and connected healthcare devices.
- Ability to work under stressful conditions.
- Ability to get to the job site.
Preferred Qualifications:
- Bilingual in English and Spanish (for some locations this is REQUIRED).
- 2 years of experience working in a retail/clinic environment (prior knowledge of local healthcare systems and health plans a plus).
- Health coaching in the areas of Diabetes, Colorectal screening, BP monitoring, and health risk assessments.
- Experience in assisting customers to resolve complex issues.
- Advanced computer and tablet skills, such as scanning and uploading documents and typing 40-50 WPM.
- Previous experience with EHR/EMR.
