HOW YOU'LL MAKE AN IMPACT
Your Role:
- Utilizes the nursing process and adult-child learning theory to assess, plan, provide, and evaluate direct patient teaching/counseling to persons diagnosed with diabetes mellitus and related health conditions.
- Coordinates and “case manages” an array of patient services (as needed).
- Liaises with and serves as the primary point of contact (POC) with referring/managing physicians, education team members, and Diabetes Center medical directors to identify diabetes education and management goals and clinical alternatives to improve patient status.
- Provides services in individual and group settings, under an RN/Assistant Nurse Manager’s supervision, at designated locations (on- and off-site).
- Plans and participates in professional training and outreach/marketing activities (as initiated and directed).
- Displays an understanding of diversity.

- Enhances and contributes to a safe environment for all colleagues, visitors, and patients.
- Performs miscellaneous duties not expressly described herein.

WHAT YOU’LL NEED TO APPLY
Required Education:
- Associate degree in nursing
Preferred Education:
- Bachelor’s degree in nursing
Required Experience:
- Two (2) years of relevant job-related experience
Required Certification/License/Registry:
- Licensed as an RN in accordance with Florida Statutes (FS), Title XXXII (Regulation of Professions and Occupations), Chapter 464 (Nursing), Part I (Nursing Practice Act).
- Certified Diabetes Care and Education Specialist (CDCES) credential from the Certification Board for Diabetes Care and Education (CBDCE) within one (1) year post-hire.
Preferred Certification/License/Registry:
- CDCES credential at hire.
