We provide a wide variety of services to adults, adolescents, children, families, schools, and communities. We are privileged to touch lives of all ages, cultures, and faiths.
MISSION IMPACT: All LCSNW team members will contribute to our mission of partnering with individuals, families, and communities for health, justice, and hope. The Case Manager assists and supports refugee, asylum, and immigrant clients to help them resettle and integrate into their new community and provide the resources and support to achieve self-sufficiency.

- Connect clients with community networks and support systems to ensure their long-term integration into the community (this may include working closely with volunteers who are supporting clients).
- Provide transportation for clients, including scheduling medical transportation or ensuring clients understand and have access to public transportation, to attend required appointments and ensure that language access is provided at these appointments.
- Provide in-home services to refugees including case management, needs assessments, cultural orientation, education, crisis support, information, and referrals as needed.
- Keep in regular contact with active cases per contractual requirements, including home visits, office meetings, community orientation, and phone calls.
- Maintain organized case files, complete accurate progress notes for all services provided, submit necessary reports in a timely manner, and keep shared tracking forms up to date.
- Coordinate needed services through referrals to refugee service providers including the Health Department, Employment Services, Social Security, DSHS, and ESL providers.
- Ensure that clients are being served in a linguistically and culturally appropriate manner and provide education and advocacy when necessary.
