The Department of OBGYN has an exciting opportunity for a Temp Patient Navigator to work in Miami, FL. The Patient Navigator facilitates patients’ ability to obtain services by identifying barriers to care, coordinating the development of treatment plans, and monitoring the implementation and follow-up of services ordered by caregivers.
Responsibilities:
- Conduct patient evaluations to obtain information necessary for the development of an effective treatment plan.
- Ensure patients have access to insurance and social service programs for which they are eligible.
- Provide patient reminders of upcoming appointments and act as liaison between patients and providers.
- Identify and resolve real or perceived barriers to care.
- Educate and counsel patients on issues related to their health and life situations.
- Maintain current knowledge of community health, welfare resources, and any other programs available to patients.
- Adhere to University and unit-level policies and procedures and safeguard University assets.
Minimum Qualifications:
- High School diploma or equivalent required.
- Minimum 3 years of relevant work experience required.
- Ability to process and handle confidential information with discretion.
- Ability to communicate effectively in both oral and written form.
- Ability to handle difficult and stressful situations with professional composure.
- Ability to maintain effective interpersonal relationships.
- Ability to understand and follow instructions.
Department Functions:
- Accompany patients to Clinic and ER visits and visit patients admitted to the hospital as needed or requested to provide support and improve adherence to care.
- Provide new patients with an orientation to the Clinic, including available services both on-site and in the community.
- Provide the clinical care team with knowledge regarding the needs and concerns of the patients and assist in planning appropriate activities to address these.
- Assist in planning promotional, educational, and other events to further the access and adherence to care.
- Distribute educational literature and disseminate information regarding available services to targeted population.
- Assist in the development and administration of a patient satisfaction survey.
- Communicate with patients effectively to identify primary concerns and provide ongoing emotional support to patients as needed.
- Facilitate communication between the patient, physician, and all members of the clinical care team.
