Case Manager (Bilingual) Job – Dallas, TX
This position is responsible for assisting clients to access medical care, psychosocial support, and the resources necessary to meet the challenges of living with HIV and working towards long term adherence to medical care and their care plans.
Responsibilities
- Provides appropriate non–medical case management for clients assigned to caseload.
- Follows case management standards and established agency procedures related to providing non- medical case management services.
- Conducts client comprehensive needs/resource assessment; participates in electronic health record case management assignment process.
- Obtains all necessary documents to provide client care.
- Maintains client confidentiality and privacy; ensures informed consent.
- Develops and implements a comprehensive care plan in collaboration with client, based on
assessment of eligibility, needs, barriers, acuity and resources; adjusts care plan as needed. - Assesses/evaluates client needs and refers clients to medical care, prevention, psychosocial and
other treatment and/or support services; follows-up on referrals to ensure linkage and directly
intervenes to facilitate provision of service where needed. - Manages case-load by maintaining contact with clients based on acuity level and protocols regarding
contact standards. - Conducts home and other off-site client assessments as appropriate.
- Apprises supervisor of challenges related to barriers to care, systems abuse, duplication of services,
and performance by referred providers, client grievances and new emerging resources. - Follows all protocols, completes accurate and timely documentation of each client encounter into
requisite databases and submits necessary reports to supervisor on time. - Identifies clients for case management graduation, inactivation, or file closure.
- Completes all required DSHS and departmental trainings for case management.
- Other duties as assigned by supervisor.
Skills & Qualifications
Case Manager Required Knowledge, Skills and Abilities:
- Bilingual/Spanish required
- Proficiency in Excel, Word, and Outlook.
- Ability to work in a positive and empathetic manner with persons who have HIV/AIDS.
- Ability to make decisions related to appropriate client care.
- Working knowledge of medical/psychosocial resources and the medical and psychosocial
complexities of HIV/AIDS. - Ability to effectively communicate in verbal and written formats.
- Ability to collaborate with community service providers.
- Ability to establish effective working relationships with various clients within the community
- Ability to manage and work effectively in the required electronic medical record, database, or
document portal. - Ability to work in a multi-site work environment
Education and Experience:- Bachelor’s degree in social work, health education, nursing or other social services discipline.
- Preferably 1-2 years of experience in case management or other social service discipline.
