Job Summary
The Licensed Practical Nurse (LPN) Clinical Care Leader (CCL), Long-Term Care (LTC) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide individualized care in the long-term care setting while under the supervision of a registered nurse, advanced practice provider, or physician.
This position's main duties include completing, updating and maintaining the schedule for our Nurses and CNAs, making and maintaining up to date schedules for appointments and physician visits for our residents, updating and ensuring accuracy of timecards for the nursing department, and leading orientation training for new hires in the nursing department. Collaborates with other inter-professional colleagues, to plan, implement, and evaluate care. Documents patient/resident history, symptoms, medications, and care provided in compliance with applicable standards/regulations.
Qualifications
Graduate from an accredited practical nursing program as a Licensed Practical Nurse. Associate degree in practical nursing is preferred. Must have attended a pre-licensure nursing program that included supervised clinical experience across the life span as part of the curriculum or have had a minimum of 400 hours of experience as LPN at another facility. Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
